Summit Spine & Wellness, LLC ("Summit," "we," "us," or "our") respects your privacy. This policy describes information associated with our website and optional text messaging, including communications supported by our electronic health record provider, ChiroTouch.
1. Information we collect
Depending on how you interact with us, we may receive:
- Contact information: your name, telephone number, and other contact details you provide when communicating with our office.
- Appointment and communication information: scheduling details, questions, and the contents of messages you send us.
- Messaging records: your mobile number, SMS consent and opt-out status, when and how consent was given, and message delivery information.
- Technical information: our website hosting and embedded service providers may process your IP address, browser information, requested pages, and access times when you visit.
2. How we use information
We use information to respond to inquiries, coordinate appointments, provide patient-service communications, maintain communication preferences, and operate and protect our website and messaging services. Consent records help us document your choices and honor requests to stop messages.
Visiting this website or giving us a telephone number does not, by itself, enroll you in recurring text messages.
4. Text messaging terms
For the full program terms, please read our Terms & Conditions.
Program: Summit Spine & Wellness patient communications. If you choose to opt in, we may send appointment confirmations, reminders, scheduling updates, and responses to patient-service inquiries. Messages may be sent using automated technology.
- Your choice: SMS participation is optional. Consent is not required to purchase services or receive care. You can contact our office by telephone instead.
- Frequency: message frequency varies based on your appointments and communications with us.
- Charges: message and data rates may apply under your mobile plan.
- Stop messages: reply STOP to a Summit text to unsubscribe. You may receive a final confirmation of your request. Further program messages will stop unless you opt in again.
- Get help: reply HELP or call (941) 907-9663. You can also call to update your number or communication preferences.
- Delivery: delivery depends on your carrier and network availability. Carriers are not liable for delayed or undelivered messages.
Please avoid sending sensitive medical or financial details by ordinary text message. Contact our office for an appropriate way to share that information. Text messaging is not an emergency service.
5. Website and third-party services
This website currently provides information about our practice and links for calling or getting directions. It does not include a patient intake form or a website SMS enrollment form.
We use Google Fonts and an embedded Google Map. Loading these features connects your browser to Google, which may receive technical information and use cookies or similar technologies according to its own policies. 6. Retention, protection, and your choices
We retain information for the purposes described here, including responding to requests, maintaining required records, and documenting messaging consent and opt-outs. Retention depends on the type of information and applicable recordkeeping obligations. Opting out of texts does not automatically delete records that must be retained.
We take reasonable steps to safeguard information, but no website, electronic transmission, or storage method can be guaranteed completely secure.
To ask about information you have provided, request a correction or deletion, or change communication preferences, contact our office. We may need to verify your identity, and requests are subject to applicable law and recordkeeping requirements.
7. Health information
This website and messaging policy is not a HIPAA Notice of Privacy Practices. The use and disclosure of protected health information, and patients' rights concerning that information, are addressed through the practice's applicable healthcare privacy notices and obligations.
